To reverse a vasectomy, Dr. Hickman will need to find the two ends of the vas, free them from the surrounding scar tissue, align them carefully and join them using a fine nylon suture and ensure a watertight closure for a higher success rate. He will use a Zeiss OPMI-MDM operating microscope to ensure a direct alignment of the two ends.

But before joining the two ends, Dr. Hickman will first collect a sample fluid from the testicular end of the vas deferens and examine it under a laboratory microscope. There are three possible scenarios. The first possibility is that the sample fluid contains whole sperm, which is the best possible situation. The second possibility, which is the most common, is that the sample fluid is thin and contains no whole sperm but instead just sperm parts. The third scenario is that the fluid is pasty and thick and doesn’t contain any sperm. This is due to an obstruction, which is known as an epididymal blowout. The obstruction is at two levels: one at the site of the vasectomy and the other due to a rupture of the tube in the epididymis.

If Dr. Hickman encounters the third scenario, he will need to perform a vasoepididymostomy in addition to the vasovasostomy. Both sites will need to be repaired to optimize the chances of success. The method to be used for the vasoepididymostomy is a microscopic invagination of the epididymal tube into the lumen of the vas deferens using nylon sutures. Patients need not worry because there will be no additional cost if both repairs have to be performed.
