Cadence Releases First Outcomes Report: Remote Patient Monitoring-Powered Chronic Disease Care Offers the Path Forward

With more than 13,000 actively monitored patients, Cadence data highlights how various patient subgroups experienced significantly improved clinical outcomes and a sizeable reduction in total cost of care

Today, Cadence, a health technology company partnered with eight leading health systems across the U.S., unveiled its inaugural Outcomes Report. The report finds that an advanced practice provider-led remote patient monitoring (RPM) program with proven treatment protocols provides a scalable solution for chronic care management nationwide.

The data shows Cadence’s remote monitoring solution is a win-win-win that: Positively impacts patient outcomes and leads to more effective primary care for chronic disease patients; reduces healthcare costs for patients, hospital systems, and the U.S. healthcare system at large; and helps physicians deliver better clinical care while also saving them time and alleviating burden.

Positive impact on patient outcomes

In partnership with leading academic medical centers and health systems in the U.S., Cadence has built a virtual heart failure clinic, virtual diabetes clinic, and virtual hypertension clinic that deliver guideline-directed care to patients 24/7 and produce world-class outcomes. For example:

  • 2x the number of patients achieved their blood pressure goal in a study of 4,006 hypertension patients from February 2022 to April 2023 across 11 states.
  • Significant increase in the number of heart failure patients on guideline-directed medical therapy (GDMT) after enrolling in Cadence. Improved adherence to all four pillars of GDMT significantly improves clinical outcomes and quality of life for heart failure patients.
  • 43% of type 2 diabetes patients achieved their blood glucose goal.

Lower cost of care

Across various analyses, Cadence patients also experienced a significant decrease in their total cost of care after enrollment. Examples include:

  • 51% decrease in patients' total cost of care, inclusive of incremental costs associated with RPM services, in a study of patients and eligible patients with heart failure, hypertension and type 2 diabetes.
  • 50% reduction in total cost of care for hypertension patients and 52% reduction in total cost of care for heart failure patients in two separate studies of enrolled and non enrolled patients.
  • 18% reduction in emergency department visits and 63% reduction in ambulance rides after six months of participation in the Cadence program.

Time saving for clinicians

Cadence’s team of nurse practitioners, registered nurses, and clinical navigators act as an extension of physician practices. Using national guidelines and mutually agreed upon clinical protocols, the Cadence team titrates medications, orders labs, and provides education and lifestyle coaching to patients. The solution is helping physicians deliver better clinical care while also saving them time. For example:

  • 59,000 alerts managed and resolved by Cadence’s clinical team, with only 0.2% requiring escalation to partner physicians.
  • No measurable impact on the volume of EMR messages in providers’ in-baskets after placing orders for Cadence.
  • 7% reduction in no-shows to partner clinics, demonstrating improved patient engagement.

Taken together, these results underscore the potential for broad-scale change by using RPM-powered virtual care to improve preventative care for patients with chronic disease.
